3. Clinical Governance
Prescription assessments available through the Website are conducted by appropriately trained, verified and certified clinicians operating under the clinical governance framework of Health Exchange UK.
Where a prescription-only product is considered appropriate, it will be prescribed only by an authorised independent prescriber acting within their legal scope of practice.
Derma Exchange Ltd does not employ or direct clinicians in relation to prescribing decisions.
4. Independent Prescribing
All prescribing decisions are made independently by the authorised prescriber.
Prescribers determine whether treatment is appropriate after considering:
- your consultation;
- medical history;
- current medications;
- allergies;
- skin condition;
- uploaded photographs (where applicable);
- previous treatments;
- contraindications;
- recognised clinical guidance.
Commercial considerations do not influence prescribing decisions.
5. No Guarantee of a Prescription
Booking or paying for a consultation does not guarantee that:
- a prescription will be issued;
- treatment will be recommended;
- a specific medicine will be prescribed;
- repeat prescriptions will be approved.
If the clinician determines that treatment is not clinically appropriate, they may:
- decline treatment;
- recommend non-prescription products;
- recommend alternative treatment options;
- advise you to consult your GP or another healthcare professional.
6. Eligibility for Prescription Products
To be considered for a prescription product, you must:
- be at least 18 years of age;
- complete the required consultation;
- provide accurate and complete information;
- provide photographs where requested;
- be located in a jurisdiction where supply is legally permitted;
- satisfy the clinician that treatment is clinically appropriate.
Additional eligibility criteria may apply depending on the specific medicine.
7. Your Responsibilities
You agree to:
- provide truthful and complete medical information;
- disclose all medications you are taking;
- disclose allergies and previous adverse reactions;
- inform the clinician if you are pregnant, planning pregnancy or breastfeeding where relevant;
- notify us of any significant changes in your health before receiving treatment;
- follow the clinician's advice and the product instructions.
Failure to provide accurate information may result in treatment being delayed, declined or discontinued.
8. Clinical Assessment Process
Before any prescription is issued, the clinician may require:
- completion of a medical questionnaire;
- a virtual consultation;
- clinical photographs;
- further information or clarification;
- previous treatment history;
- confirmation of identity where appropriate.
The clinician may also determine that additional investigations or an in-person assessment are necessary before prescribing.
9. Identity Verification
To help prevent fraud and protect patient safety, we may require you to verify your identity before a prescription product is supplied.
Verification may include confirmation of:
- your full name;
- date of birth;
- address;
- contact details; or
- other reasonable information necessary to confirm your identity.
10. Prescription Fulfilment
Where a prescription is approved, Derma Exchange Ltd will arrange fulfilment through appropriately authorised dispensing and supply arrangements, in accordance with applicable UK legislation.
Orders will only be processed after:
- prescription approval;
- payment authorisation (where applicable);
- completion of any required verification checks.
11. Repeat Prescriptions
Repeat prescriptions are not automatic.
Each request is subject to a fresh clinical review.
The clinician may require:
- an updated questionnaire;
- new clinical photographs;
- a follow-up consultation;
- confirmation of treatment response;
- details of any side effects or changes to your health.
A repeat prescription may be refused if continued treatment is no longer considered clinically appropriate.
12. Side Effects and Adverse Reactions
Prescription skincare products may cause side effects.
You should:
- read the Patient Information Leaflet supplied with your medication;
- follow the prescribed directions;
- report unexpected or serious side effects promptly;
- discontinue treatment and seek medical advice where appropriate.
If you experience a severe allergic reaction or another medical emergency, contact 999 immediately or attend your nearest Accident & Emergency Department.
13. Safe Use of Prescription Products
Prescription products should:
- only be used by the individual for whom they were prescribed;
- be used strictly in accordance with the clinician's instructions;
- not be shared with others;
- be stored in accordance with the manufacturer's recommendations;
- be kept out of the reach of children.
Misuse of prescription medicines may present significant health risks.
14. Storage and Disposal
Customers are responsible for storing prescription products safely.
Unused or expired prescription medicines should be returned to a community pharmacy for safe disposal where appropriate.
Prescription medicines should not be disposed of with household waste or flushed down sinks or toilets unless specifically advised.
15. Returns of Prescription Products
For reasons of patient safety and hygiene, prescription-only products cannot normally be returned once supplied unless:
- the product is faulty;
- supplied in error;
- damaged during delivery;
- recalled by the manufacturer;
- otherwise required by law.
This does not affect your statutory rights regarding faulty goods.
16. Misuse of Services
Derma Exchange Ltd reserves the right to refuse future consultations or supplies where we reasonably believe that a customer has:
- deliberately provided false information;
- attempted to obtain prescription medicines fraudulently;
- misused prescription products;
- abused clinicians or staff;
- breached our Terms & Conditions.
Where appropriate, suspected unlawful activity may be reported to relevant authorities.
